Attention Medical Records @ Horsham Clinic:

Seeking Contract signed & complete medical records, I believe, 8.17.2012 & intend to show my face, personally with valid ID & whatever small fee, required, although, quite frankly, I am owed an explanation, & not more fees, suits, etc.

I believe my contract was BREACHED via Seskate 1666 & I am currently operating free from FEAR & in the Solution, which is a raw foods diet, adequate fresh air & sunshine & exercise & access to program of recovery & the steps. I filed grievance upon grievance upon grievance to be filed, faxed to my lawyer @ the time, who operated in Orthodox Law & was Operating in FEAR, & to a family member, Andrea Swyvulak who is a Family Psychiatrist. I am looking into those matters presently & I can assure you, those faxes better have been sent or you will be in even more trouble than you are at the PRESENT TIME. I want the name of my Social Worker during my stay & I want to know, why she did not inform me, that when checking out AMA, due to BREACH of Contract, that I would be stuck with a ridiculous bill, when I was provided with neither healthy food self selections, adequate fresh air & sunshine, access to program of recovery meetings, etc. I was taken out to smoke & was advocating the raw foods diet at that time & certainly WAS NOT SMOKING. I would like to come in & speak on behalf of recovery programs now & you certainly will see me around in Anonymous Face. Best of luck to your system. I believe you to be in grave danger & you need to Stop. Pause. Breathe. Reflect. Shift. Act Honestly & Authentically for once & follow through on my request without shredding the documents in FEAR. I am coming in Monday morning seeking to collect those documents & will be in a place of peace & serenity. I am no longer in DANGER from you, your acute wing, your staff, your social workers or YOUR UHS FACILITY.
